Community Group Program and Trade Show Participation Guide

A RAC-BC Program

A guide that explains the benefits and the purpose of the Community Group program and participation in Trade Show events. This guide contains the Regulations associated with formation of a Group, recommendations in how to successfully paticipate in Trade Shows and Community Events, roles for participating members and guidelines on how to provide 10 minute trial sessions at events. Annual Dues Statement/ Chapter Membership Yearly Confirmation Form

The Annual Dues Statement, also called the Chapter Membership Yearly Confirmation Form is a requirement of RAC-national and must be filled out annually by B.C. Chapter members holding a Professional RAC membership. Submitting this form keeps your membership in the B.C. Chapter "active". This purpose of this statement is to ensure that your Chapter receives a portion of your Professional membership fees. In PDF formats.

Enduring Proxy Form
The Annual Dues or Chapter Membership Enduring Proxy Form - This form may be completed in lieu of the Chapter Confirmation form above, which eliminates the need for you to complete and submit the Chapter Confirmation form every year. This proxy form simply gives our B.C. Chapter Membership Director the authority to complete and submit the Statement on your behalf. In PDF format.

Client Receipt Template - RAC-BC

This template can be used by B.C. Chapter Members to produce official receipts for your clients. The template can be customized to suit your needs by inserting your practice/personal logo and information in the spaces provided.

To download the Template click here. Do not open. Save to your download folder or desired location. To move or store the Template to another location right click on the icon and copy. Select the location on your computer where you would like the Template stored. Right click and paste. To modify the Template - right click on the Template icon and click open. To start a new document from the Template without making changes to the Template double click on the Template icon.

Client Personal Information and Medical History Form - RAC-BC
The Personal Information and Medical History form provides for contact information and a medical profile of your Clients on their initial visit.
Client Session Charting Forms

The Client Session Charting Form (Word or PDF) provides for the documentation required for tracking applications during patient/client visits. This form can be customized to suit your individual needs. An add-on (Word or PDF) form provides you with additional space for documenting. Please note: These forms are intended for Reflexologists only and cannot be used by students, who must use RAC approved Charts provided by their Instructor.
